Welcome to Mynd Mgmt! I'm Nicole Spence, PMHNP-BC, with over 13 years of nursing experience under my belt (yes, I’ve worn a few different hats, and scrubs), I’m dedicated to delivering compassionate, integrative, and patient-centered behavioral health care. My background spans emergency departments, family and sports medicine, dermatology, and urgent care, so I’ve seen a little bit of everything (and probably treated it, too). All of it has deepened my appreciation for how closely mental and physical health are connected.
For the past 3 years, I’ve been serving the community as a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC), providing comprehensive psychiatric assessments and medication management, minus the judgment and clinical coldness.
My approach is rooted in genuine respect for each person’s story. I aim to create a space that feels safe, supportive, and just human enough to laugh when needed. Whether you’re navigating anxiety, depression, or just trying to make it through the week with your brain intact, I’m here to help you move toward balance, clarity, and better days ahead.

Personal Philosophy
I believe in treating the whole person not just chasing symptoms around like we are playing medical whack-a-mole. My approach is evidence-based and individualized, tailored to each person’s unique genetic makeup, medical history, and how they've responded to treatment in the past.
Diagnostic labs play a crucial role in this process. They help uncover what’s happening under the hood, because let’s face it, if your body’s running on fumes, your brain probably isn’t firing on all cylinders either. Nutrient deficiencies, hormone imbalances, and other sneaky internal issues can seriously affect mental health.
By focusing on the connection between physical and mental well-being, I create personalized treatment plans that support both mind and body. It’s holistic care that aims for long-term wellness not just a quick fix. And while mental health is serious business, I believe we can still keep it human, relatable, and even a little lighthearted along the way.

Laboratory tests in psychiatry are valuable tools for ruling out medical conditions, confirming substance use, guiding medication choices, and identifying possible underlying factors contributing to mental health symptoms. While labs are not a replacement for clinical assessment, they play an important complementary role in providing a holistic understanding of a patient's health, improving diagnostic accuracy, and optimizing treatment plans.
For comprehensive care, treatment is referred to your PCP or endocrinologist to address underlying medical conditions such as infections, autoimmune disorders, thyroid/hormonal imbalances, or metabolic disorders.
A lab order will be provided. The patient has the flexibility to have the lab work completed at any facility of their choice.

🧠 Brain & Body Medication Optimization Evaluation
A comprehensive consultative evaluation designed to better understand biological factors that may influence mental health symptoms and medication response. This evaluation integrates pharmacogenomic testing, laboratory markers, medication history, hormones when relevant, and nutritional or metabolic factors to help identify patterns that may contribute to symptoms such as depression, anxiety, ADHD, fatigue, brain fog, sleep disturbance, or medication side effects.
The goal of this evaluation is to provide additional biological insight that may help guide more personalized treatment decisions.
🇺🇸 Veteran Nexus Letter Evaluations
Independent medical opinion evaluations for veterans seeking professional clinical review related to potential service-connected mental health conditions. These evaluations involve a detailed review of medical history, service records when available, and clinical information to determine whether a medical nexus may exist between military service and the claimed condition.
When appropriate, a formal written nexus opinion may be provided to support the veteran’s disability claim.
📄 Medical Opinion for VA Claims
A nexus letter is a written medical opinion that explains the relationship between a veteran’s current medical or mental health condition and their military service.
To establish service connection, the U.S. Department of Veterans Affairs typically requires three elements:

A nexus letter addresses the third requirement by providing a professional medical opinion supported by clinical reasoning and available evidence.

Every evaluation is individualized. No template letters are used.
⭐ Early Outcomes
Veterans who have submitted nexus opinions prepared through Mynd Mgmt have successfully used these evaluations in their VA claims. However, the Department of Veterans Affairs ultimately determines claim outcomes.
